To take your Thai registered scooter over the border into Malaysia you will need to have your original green book and an English translation. You will need to get the translation before you leave, from the land and transport office, which takes up to a week.

Before Thai immigration, on the right hand-side hidden behind a row of market stalls selling dried fish and the like, there is a lady who sells Malaysian insurance which is compulsory (1200 baht for 3 months). After 1/2 hour or so she will give you your insurance papers and a filled out form to get your international circulation permit for Malaysia.

Check out of Thailand (no paperwork for the bike needed) and check in to Malaysia. Straight after the Malaysian immigration window is a door with JPJ on. Head in there and give them the papers from the insurance lady, your green book, it’s translation and some cash. You then get a certificate allowing you to take your scooter into Malaysia for 3 months.
